We carry out our mission through supportive housing, personal care, case management, STI/HIV prevention and treatment services, 340B contract pharmacy partnerships, wellness education, connections to healthcare and community resources, and our Passport to Freedom initiative supporting survivors of domestic violence, vulnerable families, and individuals working toward greater stability and independence.
At Grace House, we believe housing is more than a place to live—it can be a foundation for better health, safety, stability, connection, empowerment, and long-term independence.
OUR MISSION
Grace House mission helps vulnerable individuals and families build healthier, safer, and more self-sufficient lives within their communities. By addressing both healthcare needs and the social determinants of health—including housing insecurity, access to healthcare, trauma, social isolation, and economic hardship.
Grace House of Baton Rouge, Inc. advances this mission through senior and supportive housing, personal care, case management, STI/HIV prevention and treatment services, 340B contract pharmacy partnerships, wellness education, and connections to medical and behavioral healthcare providers. Through this integrated approach, we help individuals navigate complex health and social needs, reduce isolation, maintain independence, and decrease the risk of unnecessary institutionalization.
Through our Passport to Freedom initiative, we also provide education, resources, and supportive connections to survivors of domestic violence and vulnerable families as they work toward safety, financial stability, empowerment, and long-term independence.
