Grace House of Baton Rouge addresses the critical needs of individuals living with chronic disabilities, diseases, health conditions, and the lasting effects of domestic violence by providing supportive housing, care coordination, and access to essential health and social services that promote stability, safety, and improved quality of life. Many of the individuals we serve are older adults, individuals with disabilities, survivors of domestic violence, and vulnerable families facing challenges related to chronic illnesses such as diabetes, hypertension, HIV/AIDS, behavioral health conditions, mobility limitations, trauma, and barriers to accessing consistent healthcare and supportive services.
Our mission is to restore dignity, stability, and community by creating safe, affordable, and supportive environments where individuals can heal, maintain independence, and build healthier futures. Through supportive housing, personal care, case management, STI/HIV prevention and treatment services, 340B contract pharmacy partnerships, wellness education, and connections to medical and behavioral healthcare providers, Grace House helps individuals manage complex health conditions while reducing isolation and preventing unnecessary institutionalization.
In addition, through our Passport to Freedom initiative, we provide education, resources, and support to survivors of domestic violence, helping them achieve safety, financial stability, and long-term independence. By addressing both health-related and social determinants of health—including housing insecurity, access to healthcare, trauma, and economic hardship—Grace House empowers vulnerable individuals and families to achieve greater well-being, self-sufficiency, and lasting stability within their communities.
